Abstract

Despite belief that abuse, neglect & lack of dignity is common in institutional care for older people, hard evidence is lacking. A systematic review identified only 5 care home studies with 16% of staff admitting significant psychological abuse. Identified obstacles to appropriate care include values & motivation of practitioners, organisational factors, inadequate resources & lack of education. For many paid carers these factors result in frustration & loss of personal dignity. There is little guidance or training in relation to dignified care & lack of knowledge about recognising & dealing with abuse & neglect. The proposed study will build on work being undertaken by Tadd with the NHS workforce & will inform the prevalence study of abuse & neglect of older people in care homes. 1.Explore views & experiences of care home sector & workforce in relation to provision of dignified care & avoidance of abuse & neglect 2.Explore effect of an evidence based training intervention on attitudes & practice Objectives: 1.Identify positive & negative factors in relation to abuse, neglect & dignified care. 2.Explore views & experiences of the care home sector & workforce in relation to best practice, training, job satisfaction & wellbeing 3.Determine organisational, personal, & practice contexts in which abuse, neglect & lack of respect may occur to staff & residents 4.Undertake a preliminary evaluation of the effects of training intervention 5.Make recommendations for policy development, training & regulation in care homes Plan of Investigation The research involves: focus groups with care home owners/managers & relatives/residents exploring educational needs of the workforce. review of existing training materials observation of care practices & dynamics between residents & providers in 8 diverse care homes & completion of validated measures with staff of attitudes to older people, approaches to dementia, sense of control, job satisfaction & burnout. survey of & in-depth interviews with care staff to explore experiences & views & identify factors leading to abuse, neglect & loss of dignity development & evaluation of an evidence-based training package informed by the findings of focus groups, observations & interviews. Potential Impact The study will provide evidence to inform development & implementation of policy, legislation & practice for care home regulation, workforce training & protection of vulnerable adults. It will generate robust evidence to inform & support change identifying good practice, developing guidelines & producing of training materials. Findings will be used to: produce recommendations for training, best practice & improved care provision. identify causative factors of abuse, neglect & indignity that may indicate strategic & operational actions required for prevention of abuse, including, specific care standards for benchmarking, inspection & regulation, education & leadership development. identify staff development needs, enabling effective training.
